The ご契約状況 page. Sign in with the d ACCOUNT that pays, find Leminoプレミアム and follow the cancellation link, then the authentication and confirmation screens.

Then Lemino cannot cancel it for you. Use the store you paid through instead.

If the button does not land you there

Open lemino.docomo.ne.jp/account/contract in a browser and sign in with your d ACCOUNT
Find Leminoプレミアム under your contract status
Choose the cancellation link and re-authenticate when asked
Work through the confirmation screen to the end
If you subscribed in the iOS or Android app, cancel in the App Store or Google Play instead
